Policy Exception Request

Special circumstances may allow for policy exceptions.

  • Exceptions must be documented, justified and approved according to the Exception section of the policy.
  • Exception approvers have the right to refuse requests due to lack of sufficient justification, frequency of exception requests, lack of reasonable notice by the requestor or their judgment on the reasonableness of the exception. 

Applicable Policies

Business Services may refuse payment for noncompliance with required State, Research, or mgm casinoF funding rules and regulations, insufficient or illegible signatures.   

Complete the Form

Complete all required fields and click Submit Request. 

  • A copy of the submission data will be emailed to the requestor's email address provided on the form.
  • Print and obtain signature from both the original approver and exception approver. 
  • Submit the signed form with your reimbursement request.
